I have a domain that I have held for several years. There have not been any changes in the last 60 days so why is the "Get Authorisation Code" disabled and how do I get it enabled?

Are you certain no changes were made? Changing the registrant/owner information of the domain or moving the domain to another customer account will impose a hold on the domain.
If it's in a 60 day hold, there's no way to enable it besides waiting. If you contact Support, they'll be able to give you specific details about the hold.
